Boarding Checklist
___ Leash
___ Collar/harness with "pinch to release" buckle and my dog's first & last name on it
___ Normal food and medications packed per Woof Dah!'s "Food Packing Instructions"
___ Emergency phone number for this visit
___ 5-10 min. of time to update paperwork if my dog hasn't recently attended.
___ If my dog has been due for vaccinations since his last visit-
An updated copy of his vaccination record showing:
1.) His rabies, DHPP and bordatella vaccinations have been current for at least one week AND
2.) If he is undergoing any series of vaccinations, the series has been complete for at least one week.
___ Please be courteous in our parking area by keeping your dog quiet. Thanks!

Hello there, my name is Eeyore. I am VERY cute. If you don't notice how cute I am right away, I'll make it very clear to you that I am the cutest dog ever and therefore I should get all the attention I want. I am a teacher's pet, suck-up and mooch extrordinaire! If you have other things to do, I'll follow you around until I get the attention I deserve-I want to be held like a little newborn baby. Oh yeah, love it! Oh, and if there's a lap anywhere, um, it's mine! Don't even think on jumpin up the lap cause I'll just sit right on top of ya.... Oh, and one more thing. If you see me waiting outside the swimmin pool is cause I want my bath, NOW! Wrap in a towel to dry, please.
